Payment Policy
Payment is due at the time of service for all self-pay appointments, including insurance copay. We accept major credit cards, debit cards, and HSA/FSA cards.
Patients utilizing insurance remain responsible for all applicable copayments, deductibles, coinsurance amounts, and non-covered services as determined by their insurance carrier.
Cancellation Policy
- Cancelled more than 72 hours ahead — reschedule without penalty.
- Cancelled within 24–48 hours — may be charged 60% of the service fee.
- Same-day cancellations and missed appointments — may be charged in full.
- Repeated missed appointments may result in discharge from the practice.
