1. Telehealth Services

All clinical services are provided via HIPAA-compliant telehealth.
You must be physically located in Arizona or Texas at the time of your appointment.

2. Cash-Pay Practice

We do not bill insurance.
Payment is due at the time of scheduling.

3. No Emergency Services

We do not provide crisis or emergency care.
If you are experiencing a medical or psychiatric emergency, call 911 or go to the nearest emergency room.

4. Patient Responsibilities

You agree to:

• Provide accurate information

• Attend scheduled appointments

• Provide 24-hour notice for cancellations

• Use telehealth in a private, safe location

• Respect provider boundaries
